Discharge planning occurs at the point of admission. Once established, the resident's treatment plan will guide the treatment team toward the expected discharge goals. As the resident progresses towards accomplishment of their goals and mastery of skills necessary to successfully make the transition to the next appropriate level of care, the team will review the resident's progress. The LTSR will schedule a discharge planning meeting to review the resident's progress to date, review the discharge recommendations and assess the available of community-based treatments recommended as part of the discharge plan.
The program will accept referrals through the Base Service Unit of Beaver, and surrounding counties. Any provider seeking admission to the NSLTSR should begin the process by contacting the BSU of the client's county of origin. Upon receipt of initial documentation, the NSLTSR will schedule an admission review conference with representation for all involved parties.
